Required Documents and Information
Sole Proprietorship
- Business name filing document.
- Business license (if there is no business filing document).
- ID* and proof of address** of sole proprietor / owner.
- Proof of source of funds (e.g. most recent bank account statement).
Limited Liability Partnership (LLP)
- Certificate of Partnership registration (i.e. fictitious name, certificate of trade or assumed name certificate).
- Executed Partnership agreement / deed.
- ID* and proof of address** of at least 2 partners.
- ID* and proof of address** of all ultimate beneficial owners who have 20% or more direct / indirect of the equity interests in the entity.
- Proof of source of funds (e.g. most recent company bank account statement).
Limited Partnership (LP)
- Certificate of partnership/ business registration.
- Executed partnership agreement
- Register / list of managers.
- ID* and proof of address** of at least 2 executive officers, directors or managers.
- General Partner (GP) documents:
- Certificate of Incorporation.
- Executed Partnership Agreement.
- Proof of source of funds (e.g. most recent company bank account statement).
Limited Liability Company (LLC)
- Certificate of formation.
- Executed LLC operating agreement.
- Register / list of managers.
- Register / list of members.
- ID* and proof of address** of at least 2 executive officers, directors or managers.
- ID* and proof of address** of all ultimate beneficial owners who have 20% or more direct / indirect of the equity interests in the entity.
- Proof of source of funds (e.g. most recent company bank account statement).
Trust
- Executed Trust deed / agreement / certificate.
- ID* and proof of address** of settlor / grantor/ trustor.
- ID* and proof of address** of any protector.
- ID* and proof of address** of all beneficiaries owning more than 20%.
- ID* and proof of address** of at least 2 trustees.
- Proof of source of funds (e.g. most recent trust account statement).
Corporation (S-Corp / C-Corp)
- Articles of Incorporation / Certificate of Incorporation.
- By-Laws.
- ID* and proof of address** of at least 2 executive officers, directors or managers.
- ID* and proof of address** of all ultimate beneficial owners who have 20% or more direct / indirect of the equity interests in the entity.
- Proof of source of funds (e.g. most recent company bank account statement).
Non-Profit Organization / Charity
- Proof of Existence (i.e. Certificate of Incorporation / Registration).
- Constitution / Statute / By-Laws.
- Register / List of Directors or Board Members/ Trustees.
- Letter confirming whether there is a natural person with 20% or more interest in the assets of the institution.
- Board resolution for account opening.
- Authorized Signatories List.
- ID* and proof of address** of at least 2 directors or authorized signatories.
- ID* and proof of address** of any natural person with 20% or more interest in the assets of the institution.
- Proof of source of funds (e.g. most recent company bank account statement).
IRA / 401k
- Account agreement / adoption agreement / plan document.
- List of beneficiaries.
- ID* and proof of address** of all ultimate beneficial owners who have 20% or more direct / indirect of the equity interests in the entity.
- Proof of source of funds (e.g. most recent account statement).
* ID (i.e passport, government issued ID card, driver’s license)
